Most rental car companies allow travel to Bruny Island. However, they charge a higher insurance excess if you subsequently have an accident on a dirt road. this is the case for the whole of Tassie (or the rest of Australia for that matter). Having said that, most roads on Bruny are bitumen so unless you actually venture off road you'll be fine.
