Increased pressures at the plantar aspect of the hallux leading to chronic hyperkeratosis and ulceration is a common finding in patients with diabetes mellitus.

Hallux Valgus is considered one of the most common foot deformities, and is described as “lateral deviation of the hallux and its consequent distancing from the median axis of the body”. Treatment for hallux valgus ranges from conservative to surgical management. There are about 131 different surgical techniques.

From August 31 st, 2020 to January 1st, 2021, 6 patients with diabetic foot ulcers plantar to the hallux IPJ were treated with a selective medial band plantar fasciotomy with an 18-gauge needle. All ulcers were recalcitrant to conservative treatment of over 6 months consisting of felt pad offloading, prescription shoes, and trilaminer insoles.

Hallux valgus (HV) and plantar fasciitis are the most common foot disorders, and, to date, no study has evaluated the relationship between these disorders. The purpose of this study was to determine the incidence of and factors associated with plantar fasciitis in patients with HV deformity.

The medial band plantar fasciotomy should be used with particular caution in patient populations with morbid obesity and flat feet, and also has potential to be a surgical treatment option to offload plantar hallux ulcerations in patients with hallux limitus and low levels of activity.
